Symptom Survey
The Maestro Symptom Survey gives you a way to easily and confidentially create a very personalized report that will allow Dr. Fontana to analyze problem areas in your health and recommend specific supplements to work with your body.When you buy over the counter whole food supplements you are purchasing products that are you guessing your body needs because of symptoms you are experiencing, or because of something you read, heard from a friend or saw on the Internet. Maybe you are taking glucosamine for joint pain. Maybe you read Valerian Root is good to help with sleep. It will probably help you sleep, but it might just be a band aid for an underlying health issue.
The Symptom Survey takes out the guess work and finds the root of the problem. Once the survey is completed Dr. Fontana will analyze the information and will be able to create a specific program exactly for your body.
The Survey is FREE with no requirement to buy anything.

How can I get started?
- Contact our office for your personal password. You can do that by phone (303) 674-1500 or send us an e-mail. Please e-mail your full name, date of birth and best way to reach you in case we have questions.
- Once you receive your password you can login to the survey here: Maestro Symptom Survey
- Dr. Fontana will be automatically notified that you have completed the survey
- Our office will call you to set up a time to go over the results.
Filling out the survey….
- Each symptom has a 1, 2 or 3 next to it.
- If your symptom is Mild, occurred once or twice in the last 6 months, mark 1.
- If your symptom is Moderate, occurred once or twice in the last month, mark 2.
- If your symptom is Severe, occurred once or twice in the last week, mark 3.
- Read through the symptoms;
- If you HAVE the symptom decide if it is mild, moderate or severe and mark a 1, 2 or 3.
- If you DO NOT have the symptom then leave it BLANK and move on to the next one.
Why do I need to get a personal password?
Each survey is very detailed and is confidential so in order for us to properly process your survey we need to give you a unique password so that you are the only one who has access to your survey.
I don’t live in Evergreen, or near Evergreen Colorado, can you still help me?
Absolutely! The survey is for anyone who wants to feel better and have great information on making an informed decision about their health. We are happy to set up a phone consultation and if you decided to purchase the organic whole food supplements we recommend, we will be happy to ship them to your home.
